Complete checklist of documents required for your Poland Work Visa (National Visa – Type D) application
Your passport must be issued within the last 10 years and remain valid for at least 3 months beyond your intended stay in Poland. Ensure there are blank pages available for visa stamps.
Completed and signed Poland visa application form from the e-Konsulat online system. All fields must be filled accurately and the form must be signed by the applicant.
Two recent passport-size photographs meeting the following specifications:
Original work permit issued by your Polish employer. This document is obtained by your employer in Poland and sent to you for visa application. SP Z Accenture handles this process for you.
Official job offer letter or employment contract from a Polish employer detailing your position, salary, and employment terms. This confirms your employment arrangement in Poland.
Valid travel medical insurance policy with minimum coverage of €30,000. The insurance must cover medical emergencies, hospitalization, and repatriation for the entire duration of your stay.
Proof of visa fee payment (receipt/confirmation). The visa fee must be paid before your appointment at the embassy or visa application center.
You must submit both original documents and photocopies of all required documents. Keep your originals safe as they may be returned after verification.
The embassy reserves the right to request additional supporting documents during the visa application process. Be prepared to provide any supplementary documentation if required.
